報到事項：請依規定期限，至下列網址登錄是否願意入學就讀。

Check-In Instructions：Please indicate your willingness to enroll by the specified deadline through the following website.

(一) 報到時間：115年8月26日（星期三）起至115年8月28日（星期五）12時止。

Check-in period: From Thursday, August 26, 2026, to Tuesday, August 28, 2026, until 12:00 AM.

(二) 線上報到網址：https://forms.gle/PpJy6REsRaVZYcBn9，並將報到同意書上傳，若放棄請填寫放棄聲明書。

Please complete the online check-in at the following URL: https://forms.gle/PpJy6REsRaVZYcBn9, and upload your signed registration consent form.
If you wish to withdraw, please complete the Withdrawal Declaration Form.

(三) 逾期未報到或報考資格不符者，取銷其入學資格。

Admission will be revoked if the check-in is not completed by the deadline or if the applicant does not meet the eligibility criteria.
